Physical (Sports) Form
Marching Band is a Sport! All students must have a physical exam done by a doctor and have a form filled out allowing participation with the program. This must be completed each year.
Often these can be signed off easily if you have done a wellness checkup within the last year.
Many doctor's offices have their own form they prefer to use, but if they do not have one, you can find it here:

Private Lessons:
Parents that are interested in finding a private lesson teacher for your student, you can visit the google sheet by clicking the "PRIVATE LESSONS" link below.
Note: the names that are highlighted are names of individuals who have taught lessons before and Mr. Landis, Mr. Slabaugh, or Mr. Major either know them or know they are a good teacher.

Travel Separately Form
Please fill out the form below if you will be picking your student up from an event. Here is the process for turning in the form.
1) Parents/ Guardians can email the completed form 24 hours in advance of the event to a director or Allison Dean.
2) Parents/ Guardians can provide a printed copy at the event. This copy must be given to a director or staff. The copy must be handed to a director or staff from the parent or guardian.
